Liverpool Integrated Heart Failure Multi-Speciality Multidisciplinary Team : Liverpool University Hospitals FT, Liverpool CCG

The Liverpool Integrated Virtual Multispeciality Multi-Disciplinary Team has conducted monthly virtual MDT meetings for heart failure patients since January 2020 uninterrupted by the COVID19 pandemic. This novel service (world's first) is funded by Liverpool CCG and conducted by Liverpool University Hospitals NHS Foundation Trust. The team integrates primary care (community heart failure specialist nurses, community heart failure cardiologist, community geriatrician) with secondary care (hospital heart failure nurses, cardiologists, nephrologist, diabetologist, chest physician, palliative care consultant, geriatrician and pharmacist) and tertiary care (heart failure cardiologists and nurses) to improve management of comorbidities in heart failure patients and thereby patient outcomes.
