Diabetes Care Initiative of the Year

Optimising care, enhancing safety and supporting system resilience through collaboration - our interorganisational virtual MDT : Luton CCG (now merged into Bedfordshire Luton and Milton Keynes CCG) and Cambridge Community Services

With the system facing immense pressure during Wave 1 of the COVID-19 pandemic, and the District Nursing service near collapse, different organisations in Luton came together to form a Diabetes virtual MDT. Initially they reviewed people with Type 2 diabetes seen by District Nursing for insulin administration, many of whom had advanced frailty and appeared over-treated with high risk of hypoglycaemia. They optimised treatment for dozens of people, supported self-management, and released 43 visits/day - the workload of 4-5 District Nurses. The MDT has since developed further and supports a data-driven, population health management approach to diabetes care in Luton.
