COVID Oximetry @home : Frimley Health and Care ICS

We have built an all-in-one solution in Connected Care (which our shared care record is a part of) to proactively identify residents, enrol and monitor them in a virtual hub, and manage them at home using our PHR app. Should these patients present at hospital/GP, clinicians can view their oximetry history in Connected Care. We can also analyse data to understand symptoms and impact of interventions to improve future patient care. Our ability to identify patients has been game-changing; allowing services to proactively contact people who have tested positive for COVID-19 and are in the moderate risk category.
