A pilot project - Medicines optimisation at transfer of care for care home residents : Kent Community Health FT

The project involved a pharmacist joining the KCHFT transfer of care team, which works with the acute hospitals in east Kent.

The pharmacist reviewed patients being discharged to care homes. The aim was to establish the need for a pharmacy service to support the safe transfer of care and to carry out medication review for these patients.

Pharmacist Kirsty Matthews used a RAG rating form based on:

•  Guidance for Prescribing (2014).
