Mental Health Pharmacist in a Primary Care Network : Lancashire and South Cumbria FT
A specialist mental health pharmacist was embedded within the Fleetwood PCN for 10months on a 0.4WTE basis. The remaining 0.6WTE was in the secondary mental health setting. The pharmacist worked closely with the PCN GPs, practice pharmacists and nursing staff as well as secondary care psychiatrists, mental health nurses and practitioners. The pharmacist was able to bridge across primary and secondary care boundaries to provide quick access to specialist prescribing advice for colleagues and independently intervene in the medicines management of patients under the care of the Fleetwood PCN who may have also been under the care of secondary services.
