North-West London COVID-19 primary care remote monitoring : North West London CCGs, Hillingdon GP Confederation, Central London Healthcare CiC, Imperial College Healthcare Trust, NHSX, AstraZeneca Digital Health Team, Huma

Due to the significant challenges posed by the first wave of COVID-19, CCGs in North-West London (NWL) designed and piloted a ‘virtual-ward’ to enable better patient care, enhance clinical capacity and improve access for patients presenting with moderate COVID-19 symptoms. The pilot sought to safely monitor patients in the community by utilising scaled-up primary care hubs who provided remote monitoring through phone calls and/or a mobile application, provided by Huma. The pilot started in Hillingdon and Central London CCGs initially (covering ~730k patients), and was successfully scaled across the NWL Integrated Care System covering 2.4m patients.
