Hounslow Working Together Programme : Hounslow and Richmond Community Healthcare Trust

A step change across the local system was required in Hounslow to meet growing health and care inequalities.

This application is on behalf of a number of health and social care organisations who came together back in 2017 with a shared vision and purpose to tackle these local issues.

This collaboration is in year four of an envisaged five-year journey and the benefits to all, particularly through the recent pandemic, are both stark and remarkable.

We would like to tell our story of how we sought to make a difference to patients, families and members of the public in Hounslow.
